Permit M1990 -057 <br />Leadville Mill, Union Milling Company, LLC <br />June 13, 2013 <br />from Calais Resources to UMC. Additionally, no trespassing signs and warning signs <br />were maintained, as needed, on the property boundary (which includes the permit <br />boundary) fence. The barbed wire property boundary fence was re -wired according to <br />Colorado Department of Wildlife recommendations and as required by our Lake County <br />Conditional Use Permit. The fence was also repaired and tightened as needed. <br />4. Permit boundary stakes were maintained as required. <br />5. The mill buildings were maintained in preparation of reconditioning and restarting the <br />mill. The mill equipment continues to be operated as part of maintenance testing, but <br />no material was processed by the equipment. <br />6. Sediment control measures were performed on an as needed basis (temporary silt <br />fence construction around the ore stockpiles, temporary historic tailings storage, and <br />new tailings storage facility). Additionally, culverts were cleaned, and berms and roads <br />were maintained as required. <br />7. Ore stockpiles remain covered with visqueen material as a temporary measure to <br />minimize acid generating potential of sulfidic ores. This visqueen material remains in <br />place as long as the material remains in place. The visqueen covers were replaced in <br />October 2012 on all three low grade ore piles. Repairs were made as necessary on <br />visqueen covers in May and early -June 2013. <br />8. Weed control measures were performed as required. <br />9. UMC and Calais Resources terminated the agreement to treat Cross Mine ore at the <br />Leadville Mill. <br />10. Additional mill equipment was ordered. Construction of mill building expansions <br />continued. <br />11. The following permitting activities were performed during the year. <br />• Lake County Conditional Use Permit review was held on September 10, 2012. <br />The county commissioners unanimously approved the CUP in the public <br />meeting. <br />• An amendment to M1990 -057 to convert from a 110(2) to a 110(d) was submitted <br />on April 19, 2013. <br />• UMC filed for a Succession of Operators from Calais Resources Inc. to Union <br />Milling Company, LLC on April 19, 2013. The SO was approved on May 3, 2013. <br />• TR-4, a material change request, was submitted to CDRMS May 16, 2013, <br />addressing the material change for the tailings storage facility from a clay liner to <br />a geosynthetic clay liner. <br />Anticipated Activities: June 18. 2013 to June 17. 2014 <br />No new disturbances are planned for the Leadville Mill. Construction activities will continue <br />within the existing disturbed area to finish the tailings storage facility. Exhibit TSF 02 is <br />submitted showing the current disturbance outline and major proposed activities within the <br />current disturbance area. <br />UMC will initiate mill operations as the permits and construction allow. UMC is targeting August <br />2013 for initiating mill startup. <br />Leadville Mill activities: <br />1 CDRMS TR -4 revision changing construction materials approval anticipated July 2013. <br />2 Mill equipment, replacement and repair summer 2013. <br />3 SWMP application July 2013. (No NPDES) <br />4 MSHA compliance anticipated July 2013. <br />
