Box 620490 <br />Littleton, CO 80162 -0490 <br />303 - 947 -3499 <br />The Leadville Mill is located in the Northeast Quarter of the Northeast Quarter of Section 33, <br />and the Southeast Quarter of the Southeast Quarter of Section 28, Township 9 South, Range <br />80 West. The county designated address is 13815 Highway 24, Lake County, Colorado. The <br />owner of the Leadville Mill is: <br />Constructive Investments, LLC <br />Nick Michael, Manager <br />PO Box 620490 <br />Littleton, CO 80162 -0490 <br />Activities: June 18. 2012 to June 17. 2013 <br />Care and maintenance activities have occurred during the past year at the mill. Additionally, <br />major permitting efforts were completed at the mill and construction initiated in accordance with <br />the approved permits. Activities are reported below. <br />The mill remained idle, on care and maintenance status, with no production or milling occurring <br />in the last 12 month period. The mill completed reconstruction of the tailings storage facility <br />embankment in late September 2012 in conformance with the approved Technical Revision <br />No.3. No permanent reclamation occurred at the mill in the last 12 month period. Additional <br />interim stabilization activities continued within existing disturbed areas. Exhibit TSF 02 is <br />submitted with the Annual Report. <br />At this time, the bond for both the Leadville Mill and Hopemore Shaft totaling $81,700 remains in <br />place. The $81,700 amount was determined by the Division in November 2007. The Leadville <br />Mill reclamation estimate is $51,700 and the remaining $30,000 is for the Hopemore Shaft. An <br />Irrevocable Letter of Credit remains in place for $51,700 from the Bank of Colorado of Yuma, <br />CO for the Leadville Mill bond. <br />The Leadville Mill activities for the past 12 months are presented below. <br />1. Quarterly water quality sampling from monitoring well MW -1 continues. MW -1 had <br />insufficient water in the well during the summer, fall, winter and spring sampling <br />periods. The sampling reports are presented in Appendix 1. <br />2. Quarterly surface water quality sampling from three stations, SW -1 through SW -3, was <br />initiated in August 2012. There were insufficient stream flows to collect samples during <br />the summer, fall, winter and spring sampling periods. The surface water monitoring <br />observations are included with the MW -1 monitoring reports. <br />3. Permanent mill permit identification signs (metal - style) were maintained on the main <br />gate to the facility and on the mill building, identifying the permitee and permit number. <br />These signs were updated, as required with the approval of the Succession of Operator <br />
