Pineda <br />Re: Combination Placer, Permit Application No. M- 2013 -028 <br />Director <br />Adequacy Comments <br />Dear Mr. Vogt: <br />DRMS has completed a preliminary adequacy review of the above - referenced pen-nit application, and has the following <br />comments. <br />Application Form <br />1. As required by Rule 1.6.5(2), please provide proof that the public notice required by 1.6.2(1)(d) was published. <br />2. As required by Rule 1.6.2(1)(g), please provide the proof that the notices required by 1.6.2(1)(e)(i) and (ii) were <br />sent to the Owners of Record. This requirement would be satisfied by submitting copies of certified mail receipts <br />or proof of personal service (Rule 1.4.1(4). <br />6.3.2 Exhibit B — Site Description <br />3. Paragraph (b). Please identify the access road that leads to the site as a permanent man -made structure that is <br />located within 200 feet of the affected area, and identify its owner. <br />6.3.3(1) Exhibit C — Mining Plan <br />4. Paragraph (a). Do you wish to include a statement saying the affected lands will be used for less than 180 days per <br />year? If you do not include such a statement now, and then you operate for less than 180 days per year, you will <br />need to request a Technical Revision at that time to include such a statement in the permit application. <br />5. Paragraph (b). Please describe how the topsoil stockpile will be stabilized with a vegetative cover (seed mix, <br />application rate, and when seeding will occur). <br />6. Paragraph (f). Please specify the maximum length, width, and depth of the pit (without any backfill). Please <br />specify the height of the tailings pile. <br />7. Paragraph (i). What is the size of the drainage area that feeds each pond (in acres)? <br />8. Paragraph (i). Is the "slimes pond" part of the large pond? What is the maximum amount of "slimes" material <br />that will be stored in the pond at one time? <br />9. Paragraph (i). Please provide a demonstration that the ponds have sufficient storage capacity to contain all <br />inflows from a 100 -year storm (or snowmelt equivalent) event at a time when the pond contains the maximum <br />quantity of slimes expected to be stored in the pond. <br />Office of Office of <br />Mined Land Reclamation Denver • Grand Junction • Durango Active and Inactive Mines <br />
