Hickenlooper <br />Governor <br />Mike King <br />Executive Director <br />Loretta Pineda <br />Director <br />On March 15, 2013, the Division of Reclamation, Mining and Safety (Division) received the 110 <br />Construction Materials Reclamation Permit Application package for the Harms Pit, File No. M- <br />2013 -018. Preliminary review of the information received determined the following items must be <br />received before the Division can consider the application as being submitted and technical review <br />can begin: <br />Application Form: <br />1. The Division requires Applicants to be registered with the Colorado Secretary of State's (SOS) <br />office and that the application lists the exact name as registered with the SOS. Please register <br />your company with the SOS, provided proof of registration and revise the Applicant's name <br />on all applicable sections of the application form. <br />2. The Division requires the Applicant to provide documentation giving the signer the authority <br />to sign on behalf of the company. The authorization document may be in the form of by- <br />laws, resolution, affidavit signed by corporate secretary, certified secretary statement or a <br />notarized statement on company letterhead. <br />3. The Division did not receive a copy of Addendum 1– Affidavit of Posting Notice at Location of <br />Mine Site with the application package as required by Rule 1.6.2(1)(b). A sample of this notice <br />is attached to the application form available on the Division's website. <br />4. On Page 3, Item #11, please report the second section of Latitude /Longitude information for <br />the Primary Mine Entrance Location to two decimal places. <br />5. On Page 3, Item #11, the two versions of the Longitude /Latitude provided by the Applicant do <br />not correspond to each other.
