1.0 PURPOSE <br />The purpose of this Tailing Dam Safety Inspection and Reporting Program (Program) is to <br />establish dam safety inspection and monitoring protocols for the existing San Luis Project tailing <br />dam and impoundment (Tailing Facility). The Program outlined in this document is intended to <br />establish consistent and appropriate procedures for routine and special dam safety inspections. <br />The Program also is intended to ensure that the findings from such inspections are properly <br />evaluated, documented and communicated both internally within the Battle Mountain Resources, <br />Inc. (BMRI) organization and to the Colorado Division of Reclamation, Mining and Safety <br />(CDRMS). <br />The following sections describe the Program which consists of the following elements: <br />• Initial Formal Technical Inspection; <br />• Annual Engineer Inspections and Reporting; <br />• Quarterly and Special Inspections by Operator; <br />• Identification of dam safety related issues and resolution of dam safety related concerns; <br />• Communication protocols for dam safety related matters, including emergency <br />notification procedures; and <br />• Record keeping procedures. <br />2.0 SCOPE <br />The requirements of the Program outlined in this document apply to BMRI' s San Luis Project <br />Tailing Facility, which is located in Costilla County, approximately 4 miles northeast of the town <br />of San Luis, Colorado. <br />3.0 DAM SAFETY INSPECTION PROGRAM DESCRIPTION <br />The inspection Program presented in this section describes routine and special inspections to be <br />performed by a qualified professional engineer and on -site operations personnel. The inspection <br />Program outlines the frequency and type of inspections to be performed, and inspection <br />documentation procedures. <br />3.1. Initial Formal Technical Inspection <br />An initial formal technical inspection of the San Luis Tailing Facility will be performed by a <br />qualified registered professional engineer who is experienced in the construction and <br />maintenance of embankment and tailing dams. The initial formal technical inspection involves <br />the following components: <br />1) Compilation, Review and Assessment of Available Project Data <br />2) Initial Site Inspection <br />3) Initial Detailed Inspection Report <br />The scope of the initial formal technical inspection is summarized as follows: <br />San Luis Project <br />Tailing Dam Safety Inspection and Reporting Program <br />Page 1 of 10 <br />
