➢ CDRMS will be notified as soon as reasonably practicable, but no later than 24 <br />hours after BMRI has knowledge of failure in progress or imminent failure; <br />➢ CDRMS will be notified by telephone during regular business hours (8:00 am to <br />5:00 pm, on working days); <br />➢ If the emergency situation occurs outside regular business hours, or if the <br />CDRMS Office cannot be contacted, notice shall be given by telephone to the <br />Colorado Department of Local Affairs, Office of Emergency Management. <br />Specify to this agency, that the emergency authority is coordinated through the <br />Division of Reclamation, Mining and Safety, and to activate that Division's <br />response network. <br />• Possible Slowly Developing Failure Condition: <br />➢ Notify CDRMS no later than 24 hours after BMRI has knowledge of possible <br />slowly developing failure condition. <br />• As soon as practicable after an emergency situation or condition is reported and <br />addressed, but no later than five working days, BMRI shall provide a written report of the <br />event to CDRMS in accordance with Rule 8.2.3. <br />4.3.3 Possible Preventative Actions for Emergency Situations <br />The following actions describe some of the steps that could be taken at the tailing facility to <br />prevent or delay failure after an emergency situation is discovered: These actions should only be <br />performed under the direction of the Responsible Party, and CDRMS shall be notified of these <br />actions as soon as reasonably practical: <br />• Emergency Situation: Reduction in Freeboard (< 2 ft) on Dam Crest: <br />➢ Stop discharging water into the impoundment <br />➢ Place sandbags along the dam crest to increase freeboard <br />➢ Protect the dam slopes by placing erosion resistant materials on overtopping or <br />eroding areas <br />➢ Divert incoming floodwaters around the impoundment area <br />➢ Lower the water level by pumping or siphoning <br />• Emergency Situation: Slide on Upstream or Downstream Slope of the Dam: <br />➢ Restore Lost Freeboard by placing sandbags or filling in the top of the slide <br />➢ Stabilize slides on the slope by weighting the toe area with soil, gravel, or rock <br />➢ If necessary, lower the pond level by pumping or siphoning <br />• Erosional Seepage through the Dam Embankment, Foundation or Abutments, Connected <br />through to Pond Area: <br />➢ If the upstream end of the leak can be located, plug it or seal it with soil, <br />bentonite, plastic liner, hay bales, or other available materials <br />➢ Place sand and gravel over the seepage exit area to provide a weighted filter - <br />protected exit <br />➢ Lower the pond water level by pumping or siphoning <br />San Luis Project <br />Tailing Dam Safety Inspection and Reporting Program <br />Page 9 of 10 <br />
