4.2. Unusual Occurrence Procedures <br />4.2.1 Unusual Occurrences <br />Unusual occurrences are events or conditions that are not normally encountered during routine <br />operations and may endanger the tailing facility. Examples of unusual occurrences include, but <br />are not limited to: <br />• Large storms and flooding; <br />• Earthquakes; <br />• Significantly increased flows from the under -drain system or changes in character of <br />seepage from the drain; <br />• Fire or explosions; <br />• Human interference by terrorism, vandalism, or accident; or <br />• Substantial rise of pond level above flood storage threshold <br />4.2.2 Unusual Occurrence Procedures <br />The following steps will be taken in the event of an unusual occurrence: <br />1. Immediately report the unusual occurrence(s) to the Responsible Party. <br />2. Promptly make a special inspection and evaluate the significance of the occurrence. <br />3. Take protective or corrective actions as appropriate for the nature of the occurrence. <br />4. Activate emergency procedures (described in the following section), if necessary. <br />5. If emergency procedures are activated, inform the Colorado DRMS within 24 hours. <br />4.3. Emergency Procedures <br />4.3.1 Definition of Emergencies <br />Three categories of emergency situation are identified and defined in this Program: <br />• Failure is Imminent when: <br />➢ Freeboard on the dam crest is less than 2 feet <br />• Failure is in Progress when: <br />➢ Tailing dam crest is overtopping <br />➢ Tailing dam embankment is failing <br />• Failure is Slowly Developing or an Unusual Situation has Occurred when: <br />➢ Tailing dam behaves unexpectedly <br />➢ Strong earthquake is felt at the site <br />➢ Human interference (vandalism, terrorism, or accident) <br />4.3.2 Emergency Notification Procedures <br />The following notification procedures will be followed in emergency situations: <br />• Failure is in Progress or Failure is Imminent: Notification shall be in accordance with the <br />Mineral Rules and Regulations of the Colorado Mined Land Reclamation Board (Rules <br />8.1 and 8.2), summarized as follows: <br />San Luis Project <br />Tailing Dam Safety Inspection and Reporting Program <br />Page 8of10 <br />
