previous stipulations and conditions have been terminated or complied with and there are no new <br />stipulations added with Permit Renewal No. 6. The recent permitting actions are summarized below. <br />Permitting Actions Processed Since August 2010 Midterm Permit Review <br />One technical revision was submitted since Midterm Permit Review No. 6 (MT -6) was issued in <br />August 2010. Technical Revision No. 24 (TR -24) was submitted on August 13, 2012 to revise the <br />culvert reclamation plan for the lower portion of the Munger Canyon mine access road. This <br />revision would have allowed two large culverts to remain in place. The Bureau of Land <br />Management indicated that the culverts need to be removed and replaced with dry fords as originally <br />planned and in accordance with the approved reclamation plan. The Division requested that CAM <br />withdraw the revision and subsequently received a letter dated October 15, 2012 from J. E. Stover & <br />Associates, Inc. requesting to withdraw TR -24 (on behalf of CAM Mining, LLC). The Division has <br />withdrawn the application effective October 22, 2012. <br />There have been no bond releases since MT -6. <br />Permit Renewal No. 6 Review Process <br />On April 17, 2012 the Division notified the operator, CAM Minining LLC (CAM), that Mining and <br />Reclamation Permit C -81 -020 for the Munger Canyon Mine would expire on January 13, 2013. The <br />Division requested pursuant to Rule 2.08.5, that a renewal application be received in the Division office <br />on or before July 17, 2012. <br />CAM's application for Successive Renewal No. 6 of Permit C -81 -20 was received by the Division on <br />May 11, 2012. The application included a certificate of insurance, in addition to the required <br />application form and proposed newspaper public notice. The application was deemed complete on <br />May 17, 2012, and notices were provided to various local, state, and federal agencies on that date. <br />Public notice of the renewal application was published in the Grand Junction Daily Sentinel once a <br />week for four consecutive weeks beginning May 29, 2012. Proof of publication was provided to the <br />Division on June 29, 2012. Copies of the application were filed with the Mesa County Clerk and <br />Recorder's Office, and at the Denver and Grand Junction offices of the Division. The Division <br />received a letter dated May 29, 2012 from the State Historic Preservation Office (SHPO), which <br />included SHPO's opinion that the proposed renewal would have no effect on cultural resources. There <br />were no other written comments received from any of the local, state, and federal agencies notified of <br />the application during the review period. <br />The Division conducted an initial review of the Applicant/Violator System (AVS) versus the permit <br />application package for Munger Canyon Mine on July 15, 2012. The information in AVS was found to <br />be more current than in the permit application package. The Division's adequacy review letter was sent <br />to CAM on July 16, 2012. In our adequacy review letter CAM was requested to update Appendix A to <br />include current ownership and control information for CAM Mining, LLC and its owners and <br />controllers. CAM responded with adequacy responses on October 9, 2012 and again on October 25, <br />2012. CAM fully addressed all adequacy concerns identified in our preliminary review with their <br />5 <br />
