January 23, 2013 C-1981-014/Southfield Mine JHB <br /> final regraded bottom of reclaimed Pond 5 is between 6075 ft.and 6080 ft. Based upon the contours approved on <br /> Map 28,the eastern slope of Pond 5 would not be graded to assure retention of the GEC roadway. Final <br /> disposition of Pond 5 upon approval to remove the pond,would need to assure that this pond no longer functions <br /> as an impoundment. <br /> ROADS—Rule 4.03 Drainage 4.03.1(4)/4.03.2(4) Surfacing and Maintenance 4.03.1(5)and(6)/4.03.2(5)&(6) <br /> The Monarch Fan access road was inspected. Energy Fuels has implemented appropriate repairs to the <br /> rilling on the northern portion of the Monarch Fan Access road. The rill near Energy Fuels disturbance area <br /> boundary has been filled in, graded and additional large rock placed in the outfall to the east. Energy Fuels has <br /> established a water bar upslope from this repair. The water bar will direct runoff off the roadway. Energy Fuels <br /> also graded the rilling further south on the roadway just north of the intersection of a non-mining roadway that <br /> heads to the south west. This road appears to contribute additional runoff onto the Monarch fan road. Mr. <br /> Patterson said that this non-mining road provides access to a pasture and a landowner's cabin. Energy Fuels has <br /> established a water bar below(north)of the intersection to direct runoff to the drainage east of the roadway. Mr. <br /> Patterson inquired about adding water bars on the non-mining related road. Energy Fuels would need to keep <br /> improvements within the approved disturbance area. Any improvements not currently permitted would need to be <br /> submitted as a revision. Confirmation with the landowner regarding the proposed improvements is advised. <br /> Water bars up gradient from the intersection may effectively isolate the runoff from the non-mining related road <br /> and minimize the volume of runoff and sediment draining off the Monarch fan road. <br /> RECLAMATION SUCCESS-Rule 4.15,Rule 3: <br /> The inspection moved from MW-NW to the reclaimed portal area. Several areas that had previously been <br /> identified in DRMS inspection reports that need maintenance were observed and discussed. <br /> The slope located NW of the remaining topsoil pile and gravel pad at the reclaimed portal area has undergone <br /> some rill formation. Repair to these rills and establishment of vegetation on this slope is needed. We discussed <br /> correction measures to be taken within the permissions of Rule 4.15.7(5)(a). There is a small remaining topsoil <br /> stockpile located at the base of this slope. Mr. Patterson inquired if this topsoil could be utilized for the purpose of <br /> rill repair. The Division thought for the limited extent of repairs needed,this may be an option. Dan suggested <br /> Energy Fuels confirm with the landowner that the operator plans to conduct these repairs and utilize a portion of <br /> this topsoil stockpile. Mr. Patterson was in agreement with this idea. Mr. Patterson also inquired if excelsior <br /> blankets would be an appropriate mitigation method. The Division confirmed that we have seen success with <br /> erosion control blanket properly anchored on slopes to stabilize the soil and allow vegetation to become <br /> established. Use of a limited area of erosion control blanket would be permitted under Rule 4.15.7(5)(a). <br /> We also discussed mitigation of the slope immediately NW of the county road at this same location. Runoff from <br /> outside of Energy Fuels disturbance area boundary is running onto Energy Fuels disturbed area and adding <br /> sediment to the roadside ditch. This sediment reports to Pond 5,but rilling is starting on the sides of the ditch <br /> from this run-on. Mr. Patterson discussed armoring the ditch at the location that an undisturbed drainage meets the <br /> road ditch. Energy Fuels has durable rip rap on site. This mitigation would help slow the flow in the ditch. <br /> Establishment of vegetation on the ditch slopes would minimize the sediment as well. <br /> Number of Partial Inspection this Fiscal Year: 5 <br /> Number of Complete Inspections this Fiscal Year: 2 <br /> Page 3 of 7 <br />
