Compliance With Mine Plan N - Support Facilities Not On-site <br /> N -Other N - Special Categories Of Mining <br /> N -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a partial inspection conducted by Janet Binns and Dan Hernandez of Colorado Division of Reclamation <br /> Mining and Safety. The focus of the inspection was to verify if monitoring well MW-NW remained blocked. <br /> The weather was warm, dry and calm. <br /> Mr. George Patterson joined the inspection at 12:15 pm as the inspectors were packing up the borehole camera <br /> that was used to document the condition of the well. <br /> HYDROLOGIC BALANCE - Rule 4.05 Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5, <br /> 4.05.6; Discharge Structures 4.05.7 Ground Water Monitoring 4.05.13; 4.05.8; Impoundments 4.05.6,4.05.9; <br /> The Division used a borehole camera to observe the condition of MW-NW. The depth counter was calibrated to <br /> zero at the top of the steel casing. The top of the steel casing is 1.2 feet above the ground surface. The camera <br /> showed that the steel casing is intact. At 352.25 feet below the top of the casing the camera encountered a <br /> blockage in the borehole. This blockage does not appear changed from observations made during the September <br /> 17, 2012 inspection of MW-NW with the borehole camera. The blockage appears to be a wooden block, a short <br /> length of rope and some steel cable beneath the block. The inspector was able to get the camera between one side <br /> of the block and the steel casing. The camera was unable to view open space below the blockage. Monitoring well <br /> MW-NW remains blocked at 352.25 feet below the top of the steel casing. It is unlikely that a water monitoring <br /> probe would be unable to proceed below this elevation in the well in its current state. <br /> The Division removed the camera from the well at this time. Mr. George Patterson arrived on site and joined the <br /> inspectors at the MW-NW location at 12:15pm after the inspectors had removed the camera from the well. The <br /> Division will send a copy of the DVD recording of MW-NW to Mr. Patterson. <br /> Pond 5 was dry. Mr. Patterson discussed final grading of Pond 5,anticipating future reclamation work at such <br /> time that Energy Fuels receives approval for pond removal. Post-mining topography contours is shown on Map <br /> 28. Map 28 apparently shows the embankment roadway to the GEC gate as remaining.
