Hickenlooper <br />Governor <br />Mike King <br />Executive Director <br />Loretta E Pineda <br />Director <br />Re: Hazeltine Mine, Technical Revision Approval, Permit No. M- 2004 -031, Revision No. TR -1 <br />Dear Ms. Davis: <br />On January 16, 2013 the Division of Reclamation, Mining and Safety approved the Technical Revision application <br />submitted to the Division on May 8, 2012, addressing the following: <br />Remediation plan to lower the water table from causing mounding on the adjacent property. <br />The terms of the Technical Revision No. 1 approved by the Division are hereby incorporated into Permit No. M -2004 -031. <br />The conditions of the Division approval are noted below. Please note that all other provisions of Permit No. M- 2004 -031 <br />remain in full force and effect. <br />Stipulation # 1 <br />A technical revision must be submitted to DRMS permit #. M- 1978 -052 addressing the associated 10 inch main and power <br />line construction located within that permit area. The technical revision must be approved by the Division prior to affecting <br />any area for construction. <br />Stipulation # 2 <br />Aggregate Industries will monitor the success of the pumping plan to draw down the surface water on the adjacent <br />impacted area. The observation must be submitted to the Division on a quarterly basis and documented with photographs <br />and maps to track progress. The plan will be evaluated after one year to determine its effectiveness <br />The revised estimated liability amount of $677,338.52 required surety exceeds the $670, 602, 00 surety performance bond <br />currently held. If you have not already done so, please submit additional bond in the amount of $6,736.52.
