Inc. (NSI) Final UIC Area Permit No.0030358- 00000, effective December 4, <br />2003, authorizes injection for the purpose of solution minin(y in the NSI lease. UIC Area Permit No. 0030358 -00000 authorizes the permittee to <br />construct and operate, convert, or piuys and abandon additional injection well(s) within the area <br />permit. This well was determined to satisfy additional well criteria required by the permit. <br />This well is subject to all provisions of UIC Area Permit No. C030358- 00000, as modified and <br />as specified in the Injection Well- Specific Requirements detailed below.
