INJECTION WELL - SPECIFIC REQUIREMENTS
Well Name: 13H -I
EPA Well ID Number: C030358 -08417
Prior to Commencing Injection Operations the permittee shall submit the following information:
1. Completed Well Completion Report (EPA Form No. 7520 -9) and schematic diagram.
2. Summary of daily drilling activities.
3. Depth to the top of each formation or unit encotu-itered and any water quality or other
pertinent data that is collected.
4. Gamma Ray Log (GR), Caliper Log (CAL), Spontaneous Potential (SP) /Resistivity Log
5. Cement Bond Log (CBL) or Cement Evaluation Log
6. A successful Part I (internal) Mechanical Integrity Test (MIT)
7. Temperature Survey
8. AOR well: 13H -R
a. Well completion report
b. CBL
9. Subsurface Subsidence Monitoring Well: The DS -5 will serve to monitor subsurface
subsidence and baseline data is required.
10. Surface Subsidence Monitoring Well: 13H SSM will be placed.
Thepei7nittee shall include appropriate narrative interpretations by a knowledgeable log analyst
along with pertinent logs. Once these records are received and approved by EPA, the Director
will provide written authorization to inject.
Underground Sources of Drinking Water (USDWs)
Starting from the ground surface, USDWs in the Area Pei7nit are: a perched or unconfined
discontinuous low permeability sandstone aquifer approximately 400 feet below surface, the 16
feet thick A- Groove Aquifer, and the 290 feet thick B- Groove Aquifer.
Aquifer Exemption is not required in the Dissolution Surface Zone because the TDS of water
produced from the proposed injection well is greater than 10,000 mg /l. The L -5 zone
immediately over the Dissolution Surface has been exempted.
Approved Injection Zone
Injection is approved below the Dissolution Surface, in the L -5 zone which comprises the Boies
Bed (L -5A), L -5B, L -5C, and L -5E zones.
Confining Zone
The R -6 zone aquitard which lies directly above the L -5 zone, has been designated as the
confining interval. It is approximately 177 feet of marlstone, oil shale, and leached oil shale
between the depths of approximately 1600 ft and 1775 ft.
Maximum Allowable Injection Pressure (MAIP)
The MAIP is 300 psi as measured at the surface.
t4uthor-izatior7 for fldclitional Well: UIC Area Permit C030358 -00000
Well: 13H -1 EPA kVelI JD: C030358 -08417 Page 2 of 5
