Compliance With Mine Plan Y - Support Facilities Not On-site <br /> R - Other NA - Special Categories Of Mining <br /> NA - Processing Waste R - Topsoil <br /> COMMENTS <br /> A complete inspection of the New Horizon North Mine was conducted by me,Marcia Talvitie, <br /> of the Division, on Wednesday, December 12, 2012. Skies were overcast, and the conditions <br /> were damp and cold. Ross Gubka was available to represent the operator, Western Fuels- <br /> Colorado, LLC. Durinig the inspection I was introduced to Trevor Ragsdale, the employee who <br /> is overseeing the site development activities at the southern end of the permit area. <br /> Construction of Pond NHN-001 has been completed, and topsoil salvage operations were getting <br /> under way for the first time since being halted on October 23rd. <br /> AIR RESOURCE PROTECTION—Rule 4.17: <br /> • The contractor, Bower Brothers Construction, had two water trucks on site. Mr. Bud Bower <br /> explained that watering of the traveled routes within the permit area had been initiated in the <br /> days prior to re-commencing of the topsoil salvage operations. The water tank has been <br /> removed from the permit area, due to the lack of available irrigation water, and erected near <br /> the elevated tank at the New Horizon Mine. <br /> AVAILABILITY OF RECORDS —Rule 5.02.4(1): <br /> • Mine records were available for inspection at the operator's New Horizon Mine office. A <br /> completed DRMS Availability of Records form is attached to this report. <br /> • Permitting documents are to be made available for public review at the Montrose County <br /> Courthouse Nucla Annex. I visited the Nucla Annex at 8:45 a.m. on December 12, and saw <br /> that the permit application package, a set of four 3-ring binders, was on the shelf in the public <br /> viewing area. Other items pertaining to permitting actions that are already complete and <br /> approved were collected by me and handed to the clerk for retrieval by Western Fuels. <br /> Number of Partial Inspection this Fiscal Year: 6 <br /> Number of Complete Inspections this Fiscal Year: 2 Page 2 of 12 <br />
