The map which was submitted for Exhibit F did not conform to the requirements of Rule 6.2. <br />Please revise the map and include the following elements: <br />a) The name of the Applicant <br />b) Must be prepared and signed by a registered land surveyor, professional engineer, or <br />other qualified person <br />c) A north arrow <br />d) A map scale (not larger than 1 inch = 50 feet, nor smaller than 1 inch = 660 feet) <br />e) A map title: Reclamation Plan Map <br />3. The map which was submitted as Exhibit F contained contour lines; however, the contour <br />lines did not contain elevation labels. Please revise the map to show the associated elevation <br />of each contour line. <br />4. The permit boundary was not labeled on the Reclamation Plan Map. Please submit a revised <br />map with a clearly labeled permit boundary, see additional comments under Item No. 1. <br />6.4.12 Exhibit L — Reclamation Costs <br />5. The Division has completed the financial warranty calculation (copy enclosed) for the <br />proposed operation. Please review the enclosed figures as soon as possible and contact our <br />office if any calculation errors are noticed. <br />As previously mentioned, if you are unable to provide satisfactory responses to any inadequacies <br />prior to December 13, 2012, it will be your responsibility to request an extension of time to <br />allow for continued review of this application.
