Pineda <br />Director <br />The Division has identified several other comments and questions that must be addressed prior to <br />the Division's decision due date. Please be advised that if you are unable to satisfactorily <br />address any concerns identified in this review before the decision date, it will be your <br />responsibility to request an extension of the review period. If there are outstanding issues <br />that have not been adequately addressed prior to the end of the review period, and no <br />extension has been requested, the Division will deny this application. <br />1. The Applicant has submitted multiple maps for Exhibit C. The permit boundary as depicted <br />on the map labeled `Exhibit D — Mining Plan Map' corresponds to the property boundary, <br />except for approximately 4 acres which have been exempted from the permit boundary. The <br />permit boundary as depicted on the other maps shows that the permit boundary corresponds <br />exactly to the property boundary. Please revise permit boundary on the following maps so <br />that the permit boundary encompasses the 31 acre permit area: C(a), C(b), C(c), C(g). The <br />Division recommends the Applicant create a single map for Exhibit C and the map should be <br />labeled as `Mining Plan Map'. In addition, the map must contain the following elements: <br />Office of <br />a) The name of the Applicant <br />b) Must be prepared and signed by a registered land surveyor, professional engineer, or <br />other qualified person <br />c) A north arrow <br />d) A map scale (not larger than 1 inch = 50 feet, nor smaller than 1 inch = 660 feet) <br />e) A clearly labeled permit boundary. <br />COLORADO <br />D I V I S I O N O F <br />RECLAMATION <br />MINING <br />— &— <br />SAFETY <br />Office of <br />
