inspection under 2.07.3(4); <br />(d) The name and address of the Division to which written comments, <br />objections, or requests for informal conference on the application <br />may be submitted; <br />(e) If an applicant seeks a permit in which the affected area would be <br />within 100 feet, measured horizontally, of the outside right -of -way <br />of a public road except where mine access roads or haul roads join <br />such right -of -way line, the notice shall include a concise statement <br />describing that particular portion of the road which is located <br />within 100 feet, measured horizontally, of the affected area. The <br />notice shall also include a statement indicating that a public <br />hearing, for the purpose of determining whether the interests of <br />the public and affected landowners will be protected may be <br />requested by contacting the Division in writing within 30 days <br />after the last publication of the notice. The notice shall also state <br />that any such hearing would be held in the locality of the proposed <br />mining operation; and <br />(f) If an applicant seeks a permit involving relocation or closure of a <br />public road, a concise statement identifying the road, describing <br />the portion to be relocated or closed, the route of the proposed <br />relocation and the duration and timing of the relocation or closure. <br />The notice shall also include a statement indicating that a public <br />hearing, for the purpose of determining whether the interests of <br />the public and affected landowners will be protected may be <br />requested by contacting the Division in writing within 30 days <br />after the last publication of the notice. The notice shall also state <br />that any such hearing would be held in the locality of the proposed <br />mining operation; and <br />(g) If the application includes a request for an experimental practice, a <br />statement indicating than an experimental practice is requested and <br />identifying the provisions of those rules for which a variance is <br />requested. <br />(3) (a) Upon receipt of a complete application for a permit, a permit <br />revision, technical revision, or permit renewal, the Division shall <br />issue written notification of: <br />(i) The applicant's intention to conduct surface coal mining and <br />reclamation operations on a legally described tract of land; <br />(ii) The application number; <br />
