Compliance With Mine Plan NA - Support Facilities Not On-site <br /> NA -Other NA - Special Categories Of Mining <br /> NA -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a complete inspection of the Hamilton Mine; DRMS Permit No. C-1991-078 operated by Honeywood <br /> Coal Company. This site is located west of the town of Naturita, Colorado. I, Jared Ebert of the Colorado <br /> Division of Reclamation,Mining and Safety conducted the inspection. No one from the operation accompanied <br /> me on the inspection. <br /> AVAILABILITY OF RECORDS—Rule 5.02.4(1): <br /> A records check was conducted at the office of the Broad Canyon Dump. See the end of this report for details. <br /> HYDROLOGIC BALANCE -Rule 4.05; Drainage Control 4.05.1, 4.05.2, 4.053; Siltation Structures 4.05.5, <br /> 4.05.6, Discharge Structures 4.05.7, 4.05.10, Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water <br /> Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage —Acid and Toxic Materials 4.05.8; <br /> Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br /> An attempt was made to locate the following monitoring wells to ascertain their present condition. Monitoring <br /> wells BW 1, BW3 and AW 1 appear to be located on private property and were not accessible. The Division was <br /> unable to locate AW3 within the permit area. It appears monitoring well BW2 has been sealed and abandoned. <br /> The Division found the location of the BW2 well site and found evidence of cement work,the casing was gone <br /> and topsoil has been spread over the area. Monitoring well AW2 is still in place and has not been abandoned. <br /> The Pond B area appeared stable and vegetated. The pond was dry and it appears the tamarisk trees noted in <br /> previous inspections within the pond have been removed. <br /> REVEGETATION—Rule 4.15;Vegetative Cover; Timing: <br /> Mine Area No. 3 and 1 were stable. The perimeter fence previously in place around the disturbed area has been <br /> removed.
