Please submit the <br />remaining balance of $352.00. <br />6.4.1 Exhibit A — Legal Description <br />3. The Applicant did not provide the coordinates for the main entrance to the mine site as <br />required by Rule 6.4.1(2). The Division's records show the coordinates of the existing mine <br />site are Latitude: 38° 39' 1.49 "N, Longitude: 103° 13' 33.67 "W. Please confirm that the <br />location of the main entrance to the mine site will remain the same. <br />6.4.2 Exhibit B — Index Map <br />No comment. <br />6.4.3 Exhibit C — Pre - Mining and Mining Plan Map(s) of Affected Lands <br />4. The Mining Plan Map does not list all adjoining surface owners of record. Please confirm <br />that there are no adjoining surface owners to list or revise the map to include all adjoining <br />surface owners. <br />5. The map shows the proposed direction of mining will be in a northerly direction. Typically, <br />Operators continue to mine outward from the currently disturbed area. Please clarify if the <br />Operator will continue to mine in a southerly direction, or if mining will proceed from the <br />southern permit boundary toward the north. <br />6. As required by Rule 6.4.3(c), the Mining Plan Map must show the existing topography of the <br />area with contour lines of sufficient detail to portray the direction and rate of slope of the <br />affected land. Please revise the Mining Plan Map accordingly. <br />7. The haul road has not been included within the permit boundary. The Division reviewed <br />previous Mining Plan Maps and could not determine whether the haul road was included in <br />the permit boundary or not. The currently approved Reclamation Plan calls for reclaiming the <br />haul road when mining is complete. If the haul road is currently included 'within the permit <br />boundary, then it will need to remain in the permit boundary until it has been reclaimed. <br />Please confirm if the haul road is part of the affected area and revise the Mining Plan Map <br />accordingly. <br />8. The permit boundary, as depicted on the Mining Plan Map, encompasses 19.26 acres. The <br />application form indicates the proposed permit boundary will encompass 19.77 acres. Please <br />account for the additional 0.51 acres which have not been depicted on the Mining Plan Map <br />and revise the map accordingly. <br />9. As required by Rule 6.4.3(e), the Mining Plan Map must portray the type of present <br />vegetation covering the affected lands. Please revise the Mining Plan Map accordingly. <br />
