Compliance With Mine Plan NA - Support Facilities Not On-site <br /> NA - Other R - Special Categories Of Mining <br /> NA -Processing Waste Y - Topsoil <br /> COMMENTS <br /> A complete inspection of the New Horizon Mine was conducted by Marcia Talvitie of the <br /> DRMS on Tuesday, October 23, 2012. The inspection began at 11:30 a.m. with a records check. <br /> Ross Gubka accompanied me during the field portion of the inspection. The weather was warm, <br /> dry, and windy. <br /> PERMITTING UPDATE: <br /> SL-13 The Division issued a Preliminary Adequacy Review letter on October 17, 2012. Once <br /> all adequacy issues are resolved, the Division will recalculate the reclamation cost <br /> estimate and compose the bond release findings document. <br /> PR-07 An application for Permit Revision No. 7 was received by the Division's Denver office <br /> on October 17, 2012. A determination on the completeness of the application will be <br /> made no later than Monday, October 29, 2012. <br /> RN-06 On August 27, 2012, the Division issued a letter notifying WFC that the New Horizon <br /> Mine permit will expire on May 26, 2013. A complete application for Permit Renewal <br /> No. 6 must be received no later than November 27, 2012. <br /> AVAILABILITY OF RECORDS —Rule 5.02.4(1): <br /> • Permit records for the mine were available for review at the mine office. A completed <br /> DRMS Availability of Records form is attached to this report. <br /> • WFC submitted an updated APEN form to the Air Pollution Control Division of the CDPHE <br /> on December 10, 2010. We were not able to find a response from the APCD, which would <br /> typically be in the form of a"new" —5-year permit for the site.
