JD -6 Mine Environmental Protection Plan 4 <br />2. MAPS <br />Rule 6.4.21(2) requires the Operator to identify in the EPP on maps, sketches, and plans, the locations <br />where designated chemicals and toxic or acid - forming materials may be used, stored, handled, exposed, <br />disturbed or disposed of within the permit area, and existing or potential sources of acid mine drainage. <br />The following maps are provided in Attachment 1: <br />• Map 1 — Map Showing Lease Boundaries, Mine Permit Area, and Mine Facilities <br />• Map 2 — Environmental Protection Facilities <br />• Map 3 — Water Courses, Wells, Springs, and Reservoirs within Two Miles <br />• Map 4 — Soil Types in the Vicinity of the JD -6 Mine <br />In addition, regional and local geologic maps are provided in Section 9. <br />The coordinate system for digital base maps of the site is State Plane Colorado South (COS) NAD27 feet. <br />In the EPP, coordinates for site facilities may be reported in State Plane or Latitude /Longitude. <br />3. OTHER PERMITS AND LICENSES, INCLUDING ENVIRONMENTAL <br />PROTECTION AND MONITORING REQUIREMENTS <br />Rule 6.4.21(3) requires a listing of other agencies' environmental protection measures and monitoring, and <br />identification of which environmental protection measures and monitoring are required by statute, <br />regulation, or permit by other agencies or jurisdictions. <br />Rule 6.4.21(4) requires a listing of any air quality, water quality, solid and hazardous waste, and other <br />federal, state permits or local licenses, or other formal authorizations which the Operator /Applicant holds <br />or will be seeking applicable to the use, handling, storage, or disposal of designated chemicals and acid <br />mine drainage- forming materials within the permit area. The JD -6 Mine has the following permits: <br />• Blasting permit, <br />• Septic permit, <br />• Stormwater discharge permit from CDPHE <br />• The JD -6 mine site is zoned agricultural; therefore, Cotter has a conditional use permit (CUP) <br />issued by the Montrose County Commissioners' Office. <br />Copies of these permits are on file at Cotter's Western Slope office at 28151 DD Road in Nucla, Colorado. <br />4. FACILITIES EVALUATION <br />Rule 6.4.21(7) lists the requirements for a Facilities Evaluation. "Environmental Protection Plan Facilities" <br />are those structures that are designed, constructed and operated for control or containment of designated <br />chemicals, uranium, uranium byproducts or other radionuclides, acid mine drainage, or toxic or acid - <br />forming materials that will be exposed or disturbed as a result of mining or reclamation operations. The <br />mine facilities that would be considered Environmental Protection Plan Facilities at the JD -6 Mine are: <br />1. The existing waste rock pile; and <br />2. The existing stormwater impoundment. <br />These Environmental Protection Plan Facilities are listed in Table 2 and the locations are shown in Map 2. <br />The geochemical characteristics of the waste rock pile is described in Section 5.3 of this EPP.
