Bids will be evaluated by comparing the grand total on the enclosed Bid Schedule. For each piece <br /> of equipment, silt fence or straw bale, multiply the quantity in column C by the unit price in <br /> column E. Enter this price in column F (Total Cost). <br /> Bid Award <br /> The apparent low bidder must submit the attached Bid Schedule, signed and a certificate of <br /> insurance with the following limits: <br /> 1. Standard Workers Compensation and Employer Liability , as required by State Statute including <br /> occupational disease, covering all employees at the work site. <br /> 2. General Liability (Public Liability and Property Damage) (Minimum): <br /> a. Combined single limit of $600,000 written on an occurrence basis; <br /> b. Any aggregate will not be less than $1 million; <br /> c. The CONTRACTOR must purchase additional insurance if claims reduce the annual <br /> aggregate below $600,000; <br /> d. The State of Colorado must be named as additional insured on each comprehensive general <br /> liability policy; <br /> e. Certificates of insurance must be provided to the state within ten (10) working days after <br /> receipt of award; and, <br /> f. Insurance shall include provisions providing notification to the State ten (10) days prior to <br /> cancellation of the coverage. <br /> 3. Automobile Liability (Minimum). The CONTRACTOR must carry a minimum of$600,000 <br /> combined single limit. <br /> Project Work Description <br /> The purpose of the project is to perform routine maintenance at the mine site during snowmelt <br /> runoff. Maintenance activities which may be conducted include removal of snow, snow slide or <br /> land slide materials from roads, ditches or streams; removal of debris from culvert entries at stream <br /> crossings, maintenance of roadside ditches and culverts, and maintenance of the Dutch Creek flume. <br /> Silt fence or straw bales may be placed at various locations around the mine site to minimize <br /> sediment transportation. <br /> Work will involve removing debris from ditches, culverts, and streams under the direction of the <br /> PROJECT MANAGER. This work may occur along roadside ditches and culverts and at stream <br /> crossings. An aggregate of three hundred (300) feet of sediment containment fences (prefabricated <br /> silt fences with posts, to be installed as per manufacturer specifications, or hay bales anchored by <br /> two (2), four (4) feet lengths of 3/8 inch rebar per bale) may be constructed at various locations <br /> 2 <br />
