The Division will calculate the reclamation costs once all of the adequacy issues have been <br />addressed. <br />6.4.13 Exhibit M — Other Permits and Licenses <br />15. Please indicate if the Applicant has contacted Boulder County regarding the need for a <br />Special Use Review. <br />16. Please indicate if the Applicant has contacted the Colorado Department of Public Health and <br />Environment regarding the need to obtain a stormwater discharge permit. <br />6.4.14 Exhibit N — Source of Legal Right to Enter <br />17. The Division cannot accept the documentation which was submitted for Exhibit N. The <br />source of the Operator's /Applicant's legal right to enter must be a copy of a deed, abstract of <br />title or a current tax receipt as required by Rule 6.3.7. <br />6.4.15 Exhibit O — Owner(s) of Record of Affected Land (Surface Area) and Owners of <br />Substance to be Mined <br />No comment. <br />6.4.16 Exhibit P — Municipalities Within Two Miles <br />18. The Applicant stated there are no municipalities within two miles of the mine site. A review <br />of the municipal boundary of the Town of Lyons shows that the Stone Canyon Quarry is <br />within two miles of Lyons, CO. Please note that the Division mailed a Notice of Receipt of <br />your application to the Town Manager of Lyons, CO. <br />6.4.17 Exhibit Q — Proof of Mailing of Notices to board of County Commissioners and Soil <br />Conservation District <br />No comment. <br />
