October 6, 2012 <br />MINOR REVISION REQUEST — MINOR REVISON NO. 58 (MR -58) <br />Energy Fuels Coal, Inc. - Southfield Mine — Permit No. C -81 -014 <br />115 East Main St, Florence, CO 81226 <br />Phone 719 784 6395 <br />Fax 719 784 6395 <br />Energy Fuels Coal, Inc. (EFCI) respectfully requests approval of this submittal for Minor Revision No.58 <br />(MR -58). The revision pertains to the mitigation plan to backfill and reclaim six old subsidence holes <br />located at the former (abandoned) Canon Nation Mine site. The site is located in the SE1 /4 of Section <br />25, R7OW, T2OS of the 6 Meridian. The site is located within EFCI's permit boundaries. Owner of the <br />property is The Corley Company of Colorado Springs, Colorado. <br />The coordinates of the site are: 38 degrees, 17'01.63 "N, 105 degrees, 09'55.86 "W. <br />A map, Subsidence Mitigation Map, depicting the location of the referenced site is included with this <br />submittal. Additionally, a copy of a map generated by the DRMS' December 14, 2011 Inspection Report <br />is included which depicts the relationship of the subsidence features and mine workings. <br />Fill material: Fill material to backfill the subsidence holes will be used from the existing old spoils pile <br />that was believed to be established during the former operations of the Canon National mine. The spoils <br />pile is located adjacent to the subsidence holes. The estimated volume of fill material (compacted) <br />required is approximately 600 cubic yards. <br />Fill material transport: The fill material is located adjacent and close to the subsidence holes. Given the <br />short transport distance, a rubber tired front end loader will be used to shuttle the fill material from the <br />spoils pile to each subsidence hole. <br />Compaction: The fill material will be wheel compacted in 8 inch to 12 inch lifts during the backfilling <br />process of each hole. <br />Topsoil: Topsoil volume for the backfilled subsidence holes and the disturbed area created by removing <br />fill from the spoils pile (footprint) is estimated to be 50 cubic yards. The topsoil intended for use is <br />located at the contractor's property, Kessler Reclamation, 508 Twin Pines Ave., Rockvale, Colorado and <br />will be truck hauled to the mitigation site. Prior to use, the topsoil will be sampled and analyzed for <br />suitability according to the soil suitability criteria in the EFCI permit document. In the unlikely event that <br />the topsoil is found to be unsuitable, either another source will be tested or an approved soil substitute <br />growing medium will be used. <br />Seed mix: The previously approved Table 33, Permanent Seed Mixture and Seeding Rates for the Corley <br />property is included with this submittal. The exception to Table 33 is that the landowner, The Corley <br />Company, has designated to omit the Fourwing Saltbush from the seed mix. <br />
