,y <br /> Mr. Robert Hagen - 2 - March 4, 1991 <br /> PATTERN OF VIOLATIONS <br /> On July 17 , 1990 CMLRD determined that a Pattern of Violations existed and <br /> issued an Order to Show Cause Why Permit Should not be Suspended or Revoked. <br /> On August 10, 1990 Mid-Continent Resources requested that the Show Cause Order <br /> be vacated. Meetings were held between CMLRD and Mid-Continent Resources and <br /> on September 7 , 1990 Mid-Continent waived their right to a hearing on the <br /> matter within 60 days while the administrator considered their vacation <br /> request. At this time the matter has been scheduled for a formal public <br /> hearing before the Colorado Mined Land Reclamation Board on March 27-28 , <br /> 1991 .
