Mr. Tenore <br />Page I2 <br />October 2, 2012 <br />6.4.3 Exhibit C — Pre - mining and Mining Plan Map(s) of Affected Lands <br />2. The Mining Plan Map does not portray all adjoining surface owners of record as required by <br />Rule 6.4.3(a). Please revise the Mining Plan Map to include this information. <br />3. The Mining Plan Map does not contain a scale. Please revise the Mining Plan Map to include <br />a scale. As required by Rule 6.2.1(2)(e), the acceptable range of map scales shall not be <br />larger than 1 inch = 50 feet nor smaller than 1 inch = 660 feet. <br />4. As required by Rule 6.4.3(c), please revise the Mining Plan Map to include the name and <br />location of all creeks, roads, buildings, oil and gas wells and lines, and power communication <br />lines on the area of the affected land and within 200 feet of all boundaries of such area. <br />5. The Mining Plan Map did not contain a title. Please revise the Mining Plan Map to include a <br />title. <br />6. As required by Rule 6.4.3(g), revise the Mining Plan Map to show the owner's name, type of <br />structures and location of all significant, valuable and permanent man -made structures <br />contained on the area of affected land and within 200 ft. of the affected land. <br />7. The Mining Plan Map outlines areas of existing and future excavations. In addition, the <br />Mining Plan Map states the entire 25 acres will be bonded, which indicates the Operator <br />intends to excavate outside of the depicted excavation areas. Please revise the areas of <br />existing and future excavations on the Mining Plan Map accordingly. <br />6.4.4 Exhibit D — Mining Plan <br />8. Currently the maximum allowed disturbance at the T Bone Stone Quarry is 8 acres. The <br />Mining Plan Map states the total permitted and bonded area is 25 acres. Please confirm that <br />the Operator wants to set the maximum allowed disturbance at 25 acres. <br />9. The Division conducted a regular monitoring inspection of the T Bone Stone Quarry on April <br />25, 2012. During the inspection the Division noted the presence of noxious weeds. The <br />Operator was instructed to submit a Weed Control Plan through a Technical Revision. The <br />Operator has yet to submit a Weed Control Plan. Therefore, the Weed Control Plan must be <br />incorporated into the 112c Conversion application. The Weed Control Plan shall include the <br />following: <br />a) A list of species of noxious weeds which occur, or may occur, on the site. <br />b) A description of various control measure that will be employed to control the noxious <br />weeds such as mechanical, chemical or biological control methods. <br />c) A monitoring plan that describes how often the Operator will monitor the site for weeds <br />and what actions will be taken if weeds are found. <br />d) A post- treatment monitoring plan to determine the effectiveness of control methods. <br />
