September 18, 2012 C-1981-012/New Elk Mine JHB <br /> SIGNS AND MARKERS —Rule 4.02: <br /> NECC has updated mine ID signs to reflect new Weston, CO address. Signs need to show new mine phone number <br /> as well. Need to update information on Apache air shaft#1 and Apache air shaft 42 ID signs. Need to update phone <br /> number on ID sign on the RDA Haul road gate, at the Jansen Load Out sign, <br /> Stream buffer sign just west of the "4-way"intersection needs to be re-attached to the post to assure visibility of the <br /> signage. <br /> OFFSITE SUPPORT FACILIITES -Rule 4.04, 4.28: <br /> The Jansen loadout was inspected on 9/19. Coal has spilled onto the North side of the concrete push wall. This area <br /> is contained by a berm,but the coal needs to be cleaned up. Some additional settlement over the electrical line <br /> installation has occurred toward the east end of the concrete wall. Filling in of this settlement trench needs to be <br /> conducted. Portions of the containment berm along the north boundary of the Load Out need to be built up to assure <br /> sediment control is maintained. <br /> Steel barrels,wooden pallets and other trash need to be cleaned up. Pieces of train track and railroad ties need to be <br /> removed from the western sump and disposed of properly. <br /> Apache Canyon Air Shafts#1 and 42: ID signs are in place but need to be updated with appropriate phone number, <br /> address and all contact information. Air Shaft#1 appears stable with no evidence of erosion on the pad area or <br /> noxious weeds. Apache Canyon Air Shaft 42 exhibits some erosion adjacent to the concrete bib around the shaft. <br /> Filling in with gravel or other material is necessary to close off this gap. <br /> Monitoring wells transferred from the Golden Eagle permit to the New Elk permit were located. <br /> SF-1 was capped but needs identification marker. This well is the furthest south well that was transferred from <br /> Golden Eagle. (photo) <br /> The remaining wells are accessed through the Rancho Escondido road. NE10 and NEIOA are located just west of <br /> the Rancho Escondido road. There were no concerns with these two wells. NM-20,NM-21,NM-22, and NM-22 <br /> were all located. <br /> TOPSOIL—Rule 4.06❑ Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3, <br /> Redistribution 4.06.4: <br /> Topsoil pile located east of containment#1 needs sign replaced and this topsoil pile needs to be shown on mine <br /> maps. <br /> DOCUMENTS RECEIVED NECC provided 3rd quarter sediment pond inspection reports to the Division for <br /> New Elk Mine, and Lorencito Canyon Mine. <br /> OTHER(SPECIFY) <br /> Number of Partial Inspection this Fiscal Year: 2 <br /> Number of Complete Inspections this Fiscal Year: 1 <br /> Page 6 of 11 <br />
