RenortinQ <br /> Monitoring results for each calendar quarter are due by the 28th day of the following month. The results shall <br /> be summarized and reported on applicable discharge monitoring report forms (EPA Form 3320-1),postmarked <br /> no later than the due date. If no discharge occurs during the reporting period "No Discharge" shall be <br /> reported. Duplicate signed copies of these and all other reports required herein, shall be submitted to the <br /> Regional Administrator and the State at the following addresses: <br /> Colorado Department of Health <br /> Water Quality Control Division <br /> WQCD-PE-B2 <br /> 4300 Cherry Creek Drive South <br /> Denver, Colorado 80222-1530 <br /> Attention: Permits and Enforcement <br /> U. S. Environmental Protection Agency <br /> One Denver Place <br /> Suite 500 _ <br /> 999 18th Street <br /> Denver, Colorado 80202-2405 <br /> Attention: Water Management Division-Compliance Branch <br /> 8WM-C <br /> Failure to submit the reports shall constitute a violation of the permit and grounds for revocation of the <br /> permittee's certification to discharge under this permit. <br /> 3. Test Procedures <br /> Test Procedures for the analysis of pollutants shall conform to regulations published pursuant to Section 304 (h) <br /> of the Act, and Colorado State Effluent Limitations (10.1.5),under which such procedures may be required. <br /> 4. Recording of Results <br /> For each measurement or sample taken pursuant to the requirements of this permit, the permittee shall record <br /> the following information: <br /> a. The exact place, date, and time of sampling; <br /> b. The dates the analyses were performed; <br /> C. The person(s) who performed the analyses; <br /> d. The analytical techniques or methods used; and <br /> e. The results of all required analyses. <br /> 5. Calculations for all limitations which require averaging of measurements shall utilize an arithmetic mean unless <br /> otherwise specified. <br />
