PART I <br /> Page 3 <br /> Permit NO.: COG-070000 <br /> A. COVERAGE UNDER THIS PERMIT <br /> 1. In order to be considered eligible for authorization to discharge under the terms and conditions of this permit, <br /> the owner, operator, and/or authorized agent of any facility engaged in construction dewatering, and desiring to <br /> discharge must submit a completed application form obtained from the Water Qualify Control Division. Such <br /> application shall be submitted at least thirty (30) days prior to the anticipated date of first discharge to: <br /> Colorado Department of Health <br /> Water Quality Control Division <br /> WQCD-PE-B2 <br /> 4300 Cherry Creels Drive South <br /> Denver, Colorado 80222-1530 <br /> Attention: Permits and Enforcement <br /> The permit issuing authority shall have up to thirty (30) days after receipt of the above application to request <br /> additional data and/or deny the authorization for any particular discharge. Upon receipt of additional <br /> information the permit issuing authority shall have an additional 30 days to issue or deny authorization for any <br /> particular discharge. <br /> If the person proposing the discharge does not receive a request for additional information or a notification of <br /> denial from the permit issuing authority within the specified time frame, authorization to discharge in <br /> accordance with the conditions of the permit shall be deemed granted. Authorization to discharge shall be site <br /> specific and not transferable to alternative locations. - <br /> If the permit issuing authority determines that the operation does not fall under the authority of the general <br /> permit, then the information received will be treated as an individual permit, discharge is not allowed until 180 <br /> days after the application has been received. <br /> Authorization to discharge under this general permit shall commence on October 1, 1993 and shall expire on <br /> September 30, 1998. The Division must evaluate this general permit once every five (5) years and must also <br /> recertify the applicant's authority to discharge under the general permit, at such time. Therefore, a permittee <br /> desiring continued coverage under the general permit must re-apply by April 1, 1998. The Division will <br /> determine if the applicant continues to operate under terms of the general permit. An individual permit will be <br /> required for any facility not reauthorized to discharge under the reissued general permit. For facilities wishing <br /> to terminate authorization under the new permit, provisions of Part H.B.5 will be appropriate. <br /> 2. This permit does not constitute authorization under 33 U.S.C. 1344 (Section 404 of the Clean Water Act) of <br /> any stream dredging or filling operations. <br /> In order to be considered eligible for authorization to discharge under the terms and conditions of this permit, <br /> the owner, operator, and/or authorized agent of any facility desiring to discharge must submit, by certified <br /> letter, the discharge application form, available from the Water Quality Control Division, which generally <br /> requires the following information: <br /> 1. Name, address, and descriptive location of the facility; <br /> 2. Name of principal in charge of operation of the facility; <br /> 3. Name of water(s) receiving the discharge(s); <br /> 4. Description of the type of activity resulting in the discharge including the anticipated duration of activity <br /> and/or the discharge, anticipated volume, and rate of discharge, and the source of water which is to be <br /> discharged; <br />
