Metal Mining SWMP Guidance Document <br /> Page 15 <br /> a. Documentation of Permission: The plan shall include documentation of permission from the <br /> land owner, if the remediation work is undertaken by someone other than the land owner. <br /> In cases when remediation is being performed by an entity other than the land owner, written permission from <br /> the land owner for the remediation project should be submitted to the Division with the SWMP. <br /> b. Photographs: The plan shall include photographs documenting the condition of the inactive <br /> mine site before any remedial action has occurred. For recently active mines, photographs of <br /> the active mining phase may be substituted. <br /> Photographs should adequately portray significant potential pollution sources at the site before remedial actions <br /> begin. These photographs will be used to asses the magnitude of the remediation project and to track the <br /> progress of the project. <br /> c. Description of Remedial Action: The plan shall describe the remedial action that will take place <br /> to minimize or eliminate the water quality impacts from the mine waste. <br /> The description of remedial action is a narrative statement of the goal of the remediation project and the general <br /> scale of the remediation based on existing conditions. <br /> The description shall include: <br /> - Remediation Goal Statement, <br /> The statement of the project goal should summarize the planned tactics for handling the various pollution <br /> sources at the site. This should also include any plans for how the remediation may facilitate future land uses, <br /> as well as a proposed schedule for the project, including major benchmarks for remediation activities. <br /> - the characteristics of the mineralogic content of the mine-waste, and estimated volume of <br /> the waste; <br /> This section should include a summary of any available information on the metals, and other pollutants capable <br /> of being leached or eroded from the mine-waste, including any acid-generating materials. An estimate of the <br /> total volume of material to be remediated should be divided into separate estimates for material having <br /> significantly different compositions and final remediation goals. <br /> - description of any materials that will be mixed into the mine waste on site; and <br /> Any materials that will be imported to the site to be mixed with the mine waste should be noted (including <br /> manure, fertilizer, fly ash, etc.). If appropriate, BMPs that will be implemented to reduce the potential of <br /> imported material to contribute pollutants to stormwater discharges should be developed and noted in the <br /> "description of BMPs" section below. BMPs should address imported material stockpiles, use, and final <br /> stabilization, as appropriate. <br /> - estimated surface area of the final site, and estimated angle of side slopes. <br /> Provide an estimate of the final surface area that will be affected by the remediation project and an estimate of <br /> the angle of the proposed side slopes. <br />
