August 15, 2012 C- 1981- 044/Williams Fork Mines ZTT <br />Ponds 9 -P 1, 9 -P2, 9 -P4, 9 -P5 and 9 -P6 were all dry with no discharge. No signs of erosion were observed and all <br />embankments were stable and vegetated. <br />Culverts inspected along the Haul Road between the facility area and Pond 9 -P5 was clear of debris. <br />41, 744 <br />Rodent hole maintenance on Pond 5 -P5 <br />GENERAL MINE PLAN COMPLANCE: <br />Maintenance was performed on the southern embankment of Pond 5 -P5. Rodent holes were filled in. <br />Weed spraying that occurred in the previous months continue to show success. <br />SIGNS AND MARKERS — Rule 4.02: <br />Topsoil piles inspected were clearly marked. <br />Mine ID signs were found at the mine's main entrance as well as by the old side entrance east of Highway 13 and <br />at the entrance of the old strip pit area. All signs were in compliance with all required information stated on the <br />sign. <br />TOPSOIL — Rule 4.06 <br />Removal 4.06.2 Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4: <br />Topsoil piles north of Pond 9 -P6, east of Pond 9 -P5 and along facility area were clearly marked and well <br />vegetated.
