Lou Brekel as Director <br />beginning on 01/01/2003. Tony Casados as Chairman of the Board beginning on 01/01/2010 and Director <br />beginning on 11/01/2004. Wayne L. Child as Director beginning on 01/01/1983. Louis Costello as <br />Director beginning on 05/01/1999. Thomas Holgerson as Director beginning on 12/01/2008. Charles <br />Monk as Director beginning on 04/02/2002. Jim C. Soehner as Director beginning on 04/01/1991. These <br />officers should either be added to Tri State Generation & Transmission Assoc. list or information on their <br />end -dates should be provided to the Division. Western Fuels- Colorado LLC is listed in AVS as 99% <br />owner but not listed on the Directors /Officers Report. <br />6. For Pacificorp, the following person was listed on AVS but not listed on the Directors /Officers Report: <br />Dean S. Brockbank as Vice President beginning on 08/30/2007 and General Counsel beginning on <br />08/30/2007. This officer should either be added to Pacificorp list or information on his end -dates should <br />be provided to the Division. Centralia Mining Co. and Pacific Minerals Inc. are listed in AVS as 100% <br />owners but not listed on the Directors /Officers Report. Energy West Mining Co. and Glenrock Coal Co. <br />are listed in AVS as 100% shareholders but not listed on the Directors /Officers Report. William Fork Co. <br />is listed in AVS as 19% Shareholder but not listed on the Directors /Officers Report. <br />7. Per Rule 2.03.4 (3) (a) we need telephone numbers for the following: <br />a. Pacificorp <br />1.2.2.1; Trapper Mine <br />8. This paragraph under this section needs to be updated to include the approval of the last permit term. <br />9. Page 1 -9, Table 1.2 -1 indicates the NPDES permit expired on May 31, 2009. The Division is aware an <br />application for the renewal of this permit was submitted to the Colorado Department of Public Health and <br />the Environment ( CDPHE) and that Trapper has been working recently with CDPHE to renew this <br />permit. Please indicate the status of this process or indicate if the permit has been renewed. If so, please <br />revise the table to include the new expiration date and provide the Division with a copy of the new permit. <br />10. Page 1 -9, Table 1.2 -1 indicates the explosive license issues by the Federal Explosives Licensing Center, <br />(ID No. 5 -CO- 081- 20 -1D- 00282) expired on April 1, 2011. Has this license been renewed? If so, please <br />revise the table to include the new expiration date. <br />1.4 Determination of Bond <br />11. This entire section, tables, referenced maps, etc. need to be updated for the next permit term for the <br />projected worst case disturbance. <br />1.6 Compliance Record <br />12.
