Pineda <br />Director <br />The Division has identified several other comments and questions that must be addressed prior to <br />the Division's decision due date on September 5, 2012. Please be advised that if you are <br />unable to satisfactorily address any concerns identified in this review before the decision <br />date, it will be your responsibility to request an extension of the review period. If there are <br />outstanding issues that have not been adequately addressed prior to the end of the review <br />period, and no extension has been requested, the Division will deny this application. <br />1. The Division received comments from the Colorado Historical Society, the Division of <br />Water Resources and Army Corps of Engineers. The letters are attached for your review. <br />Please address the comments noted in the letters, and make any changes in the application as <br />necessary. <br />6.3.5 Exhibit E — Map <br />2. The revised Mining and Reclamation Plan Maps which were submitted on July 24, 2012 <br />were not labeled. As required by Rule 6.2.1(2)(e), please include a map title on both maps. <br />John W. The revised Mining and Reclamation Plan Maps which were submitted on July 24, 2012 did <br />not include labeled permit boundaries. Please revise both maps to include labeled permit <br />boundaries. <br />4. The revised Mining and Reclamation Plan Maps which were submitted on July 24, 2012 did <br />not include the access road. The access road is considered to be affected area and must be <br />included within the permit boundary. According the Division's records, the access road <br />accounts for 0.66 acres of the total 7.76 acre permit area.
