Please clarify this discrepancy and revise the text on the Reclamation Plan Map accordingly. 25. The Mining and Reclamation Plan Maps state the Right of Way (ROW) for Thompson Park Rd. is 60 feet. Please clarify if ROW is 60 feet from the center of the road. 6.3.6 Exhibit F — List of Other Permits and Licenses Required No comment. 6.3.7 Exhibit G — Source of Legal Right to Enter No comment. 6.3.8 Exhibit H — Municipalities Within a Two Mile Radius No comment. 6.3.9 Exhibit I — Proof of filing with County Clerk 26. Due to the amount of time that has elapsed since the application was submitted, please demonstrate that the application is still on file with the local County Clerk and Recorder for public review. If the application is no longer on file, then the Applicant will be required to submit a new copy of the application to the County Clerk and provide the Division with an affidavit or receipt indicating the date it was submitted. 6.3.10 Exhibit J — Proof of Mailing of Notices to Board of County Commissioners and Soil Conservation District 27. The Applicant has submitted copies of the damage waiver agreements which were provided to owners of structures within 200 feet of the affected area. The Division has since approved a standardized Structure Agreement form (copy enclosed) which must be used. Please provide all owners of structures within 200 feet of the affected area with the new Structure Agreement forms.
