5 (FORM 2 - Public filing)
G. Total project area to be disturbed 193.79 (acres)
K. Describe the equipment to be used for the prospecting operations:
Prospecting equipment is described in each of the existing DRMS NOI's to be covered under this Ground Water
Discharge NOI (See attached Table 1). In general, a water well type drill rig will be used in the drilling operations.
be used to clear the locations and build the drill pads and containment/infiltration pits. If needed, water used for
drilling and other activities will be hauled by truck to the drill sites from the nearest available source. well, or
public water cyctems. Other equipment may include a cement truck and light -duty vehicles.
L. Describe and locate any structures to be constructed (i.e. stockpiles, ponds, impoundments):
Structures associated with the prospecting sites are described in each of the existing DRMS NOI's to be
covered under this Ground Water Discharge NOI (See attached Table 1). In general, multi -hole drill
pa have bull material bluck blabh bl uckpil e( b ) di brubh / woody bit aril conta /
infiltration pits for ground water produced during drilling. Certain pads in cut/fill areas have spoil
stockpile(s). Soil material stockpiles will be covered with erosion control material. The small alluvial drill
pads typically require minimal brush clearing only during construction.
M. Describe anticipated relationship to surface water and groundwater (proximity to streams, penetration of ground
water aquifers):
Hydrology holes are completed in various ground water bearing intervals including the alluvium, Uinta
formation, A groove, B groove, L5, L4, L3, and L1. Refer to the attached general location map for proximity
of drill pads to streams and other sill face "rater bodies.
IV. OPERATION AND RECLAMATION MEASURES:
1. The Board suggests that a photographic record of the pre - prospecting and post - prospecting conditions be kept by the
prospector. These photos should be taken from the same location and by the same method to clearly show the pre - prospecting
condition of the land and the reclamation efforts. Upon completion of reclamation and request for bond or surety release, the
Board may consider the photos as evidence of adequate reclamation, and thus, be able to act more quickly on the request for
release.
2. Provide a description of the native vegetation of the area to be disturbed, including tree, shrub, and grass communities of the
area. Color photographs, sufficient to adequately represent the ecology of the site and adequately labeled (including date,
orientation and location), may be used in lieu of a written description. Based on the quality of the photographs, the Division
may require additional detail.
All drill pads and access roads covered under this NOI are located within sagebrush parks and
pinyon /juniper woodlands. The upland sagebrush is fairly uniform across most of the drill pads and access
roads.
