5) The mill buildings were maintained in preparation of reconditioning and restarting the <br />mill. The mill equipment continues to be operated as part of maintenance testing, but no <br />material was processed by the equipment. <br />6) Sediment control measures were performed on an as needed basis(temporary silt fence <br />construction around the ore stockpiles). Additionally, culverts were cleaned, berms <br />maintained and roads maintained as required. <br />7) Ore stockpiles remain covered with visqueen material as a temporary measure to <br />minimize acid generating potential of sulfitic ores. This visqueen material remains in <br />place as long as the material remains in place. Thevisqueen cover was replaced in June <br />2011 on all three piles. The temporary "historic tailings" stockpile was also covered with <br />visqueen to minimize acid generating potential of sulfitic ores. As per the Technical <br />Revision, this temporary stockpile material will be placed in the tailings facility once <br />construction activities are completed. <br />8) Weed control measures were performed as required. <br />9) Calais Resources requested Union Milling Company to reactivate the mill and in <br />accordance with that request, the following items occurred in the past 12 months. <br />a. Air Quality APEN permit applications, submitted to CDPHE in February 2011, <br />were approved and permits 11LK1369F and 11 LK 13 71 were issued. <br />b. CDOT access permit application package, submitted in May 2011, was approved <br />and permit 311081 was issued. <br />c. A construction "Water General" water supply structure was installed within <br />Leadville city limits in accordance with the Parkville Water District water use <br />agreement. <br />d. TR -3 Milling Plan, submitted in June 2011, addressing the mill configuration, <br />tailings facility reconstruction, and addressing four of five stipulations to the <br />active mine permit was approved in September 2011. <br />e. Lake County Conditional Use Permit (CUP) application package, submitted in <br />June 2011 was approved in September 2011. <br />f. Construction SWMP COR031191 approved in September 2011 and renewed in <br />May 2012. <br />g. Ordered additional mill replacement equipment and new equipment. <br />h. Grading, gravelling onsite roads. <br />i. Constructed mill building expansions, water and sewage connections. <br />j. Constructed lower access road on south side of mill. <br />k. Constructed temporary clay pad on lower access road for temporary historic <br />tailings storage during construction. <br />1. Removed historic tailings from existing storage facility and placed on temporary <br />clay pad. <br />m. Clear cut /logged trees on 1.5ac. Cleared and grubbed organic matter. <br />n. Removed topsoil and stored in topsoil storage pile. <br />10) Line of credit working capital financing was secured for start-up operations. <br />Annual Report <br />M1990 -057 June 11, 2012 <br />
