No map is submitted for Hopemore Shaft activities with the Annual Report. Total bond on the <br />properties remains at $81,700 as determined by the Division in November 2007.The Hopemore <br />Shaft reclamation estimate is $30,000 and an Irrevocable Letter of Credit remains in place for <br />$30,500 from the First Mountain Bank of Leadville, CO. <br />The Hopemore Shaft activities for the past 12 months are presented below. <br />1) The Hopemore Shaft continued operation as a tourist mine, but did not produce ore as an <br />active mine. <br />2) Permanent metal mine permit identification signs were maintained at the entrance to the <br />facility and on the mine building, identifying the permitee and permit number. <br />3) Permit boundary stakes were maintained as required. <br />4) Rehabilitation of the underground levels continued for tourist access. <br />5) Construction of board walks, parking and view deck continued. <br />6) Sediment control measures were performed as required (cleaning culverts and <br />maintaining berms and roads at the entrance of the facility). <br />7) Weed control measures were performed as required. <br />8) The Hopemore shaft does not meet the APEN submittal requirements as a tourist mine. <br />Leadville Mill <br />The mill remained idle, on care and maintenance status, with no production or milling occurring <br />in the last 12 month period. The mill initiated reconstruction of the tailings storage facility in <br />late September 2011 in conformance with the approved Technical Revision. No permanent <br />reclamation occurred at the mill in the last 12 month period. Interim stabilization activities <br />continued within existing disturbed areas and new interim stabilization activities were initiated <br />within the tailings storage facilities reconstruction area. The mill disturbed area was increased <br />from 6.6 acres to 8.1 acres. Exhibit TSF 01 is submitted with the Annual Report. Total bond on <br />the properties remains at $81,700 as determined by the Division in November 2007. The <br />Leadville Mill reclamation estimate is $51,700 and an Irrevocable Letter of Credit remains in <br />place for $51,700 from the Bank of Colorado of Yuma, CO. <br />The Leadville Mill activities for the past 12 months are presented below. <br />1) Quarterly water quality sampling from monitoring well MW -1 continues. MW -1 had <br />insufficient water in the well during the summer, fall, winter and spring sampling periods. <br />The sampling reports are presented in Appendix 1. <br />2) Quarterly surface water quality sampling from three stations, SW -1 through SW -3, was <br />initiated in August 2011. There were insufficient stream flows to collect samples during <br />the summer, fall, winter and spring sampling periods. The surface water monitoring <br />observations are included with the MW -1 monitoring reports. <br />3) Permanent metal mill permit identification signs were maintained on the main gate to the <br />facility and on the mill building, identifying the permitee and permit number. <br />Additionally, no trespassing signs and warning signs were maintained,as needed, on the <br />property boundary (which includes the permit boundary) fence. The barbed wire <br />property boundary fence was repaired and tightened as needed. <br />4) Permit boundary stakes were maintained as required. <br />Annual Report <br />M1990 -057 June 11, 2012 <br />
