Star Mine Operations, LLC Revenue Mine MCP Plan 2012 <br />1.2 Professional Engineer Certification (40 CFR 112.3(d)) <br />The undersigned Registered Professional Engineer is familiar with and has visited and examined the <br />facility, or has supervised examination of the facility by appropriately qualified personnel. The <br />undersigned Registered Professional Engineer attests that this Material Containment Plan has been <br />prepared in accordance with good engineering practice, including consideration of applicable industry <br />standards; that procedures for required inspections and testing have been established; and that this Plan <br />is adequate for the facility. <br />This certification in no way relieves the owner or operator of the facility of his/her duty to prepare and <br />fully implement this MCP Plan. This Plan is valid only to the extent that the facility owner or operator <br />maintains, tests, and inspects equipment, containment, and other devices as prescribed in this Plan. <br />1.2.1 Required Improvements <br />The Professional Engineer's certification of this plan is contingent on the following facility <br />improvements being implemented for compliance with MCP: <br />1) A full complement of emergency spill kit items must be placed near the change <br />room and tanks for use on the entire site. This kit should include a 50+ gallon <br />wheeled container with absorbent socks, pillows, pads, gloves, goggles, PPE, <br />disposal bags, ties, manual and labels. <br />2) Training of personnel must occur per Section 3.7 of this plan and designated <br />employees must become familiar with the spill kit items, their use and proper <br />reporting in the case of a spill. <br />Signature: <br />Name: <br />State and Registration #: <br />Company: <br />Date: <br />4 <br />
