Page 4 of 5 <br />25 Avenue Site <br />AM -01 Adequacy Review <br />the reclamation plan by the submittal of a technical revision, per Rule 1.9, and completed prior <br />to any full reclamation release request being considered by the Division. <br />20. In the Topsoil and Revegetation section, the Applicant provides a basic weed control plan. <br />Please state if the 25 Avenue Site will be included in the City of Greeley Noxious Weed <br />Program. <br />If the site will not be included in the City program, please submit a weed control and <br />management plan in accordance with Section 3.1.10 (6) of the Rule. This plan should be <br />developed in consultation with the county extension agency, or weed control district office and <br />should include specific control measures to be applied, a schedule for when control measures <br />will be implemented and a post- treatment monitoring plan. <br />21. In the Topsoil and Revegetation section, the Applicant states seeded areas will be covered with <br />certified weed -less mulch at a rate of 2,000 pounds per acre. Please commit to covering <br />seeded areas with 4,000 pounds (2 tons) of mulch per acre. <br />22. Please explain the backfilling process, methods and any compaction efforts required to backfill <br />the NE and SE ponds. <br />6.4.7 Exhibit G - Water Information <br />23. Please revise the acreage totals in the last sentence of the first paragraph to reflect the <br />acreages listed in the permit application. <br />24. Please commit to provide the Division with quarterly monitoring well data, at minimum, as part <br />of the annual report for the site and label the monitoring wells on Exhibit C -4 — Water <br />Resources. <br />6.4.13 Exhibit M - Other Permits and Licenses <br />25. Please commit to providing the Division with copies of the approved permits and licenses <br />required for the 25 Avenue Site amendment. <br />6.4.14 Exhibit N - Source of Legal Right to Enter <br />26. In Exhibit N and in other Exhibits in the amendment package, the Applicant states the <br />amendment area is owned by "Cottonwood Land and Farms, LLC" or "Cottonwood Land and <br />Farms, LTD." Please explain this discrepancy and correct the issue as needed. <br />6.4.18 Exhibit R - Proof of Filing with County Clerk and Recorder <br />27. Please provided an affidavit or receipt indicating the date on which the revised amendment <br />application information required to address this adequacy letter was placed with the local <br />County Clerk and Recorder for public review, pursuant to Subparagraph 1.6.2(1)(c). <br />4 <br />
