A Stormwater Management Plan (SWMP) has been developed for the site and is present on site. <br />After the proposed mine plan and stormwater controls are approved by DRMS and CDPHE, the <br />plan may be revised. This plan, which is required by WQCD, addresses both permanent and <br />temporary best management practices as well as stormwater monitoring. <br />Star Mine Operations plans to submit an Air Pollution Emission Notice (APEN) to the Colorado <br />Air Pollution Control Division (APCD) at the same time as the 112d Permit Application is <br />submitted. This notice will include provisions for controlling fugitive dust from surface activities, <br />roadways and stockpiles. <br />A solvent - cleaning station may be installed in one or both of the maintenance shops. These stations <br />generally consist of a cleaning sink mounted on a small drum of solvent. The solvent is pumped <br />into and used in the sink and then recycled back to the drum via a gravity drain. The drums of <br />solvent are periodically replaced by a vendor that recycles the solvent. If these stations are installed, <br />SMO will acquire the necessary permit(s) from the Colorado Hazardous Materials and Waste <br />Management Division (HMWMD). The electrical transformers to be used on site will not contain <br />Polychlorinated Biphenyls (PCBs). <br />4. Other Agency Permits <br />Other permits required for this operation that will include environmental protection measures are <br />listed below. Some of these permits have not yet been obtained. <br />1. CDPHE Colorado Discharge Permit System Permit with the Water Quality Control Division <br />2. CDPHE Fugitive Particulate Emission Permit with the Air Pollution Control Division <br />3. CDPHE Stormwater Management Plan with the Water Quality Control Division <br />4. EPA Spill Prevention Control and Countermeasures Plan (SPCC) <br />5. Mine Safety Health Administration, Mine Permit (This permit is completed) <br />6. USACOE 404 permit for disturbance of wetlands <br />7. Ouray County Building Permit for buildings constructed on site <br />8. Ouray County Health Department Individual Sewage Disposal System Permit (may utilize <br />porta- potties and no permit may be needed) <br />Revenue Mine August 2012 T - 20 <br />
