June 20, 2012 C-1981-044/Williams Fork Mines ZTT

inspection. No signs of erosion were observed and all embankments were stable and vegetated.

Ponds 9-P7 and 9-P8 were dry. Pond embankments were slightly vegetated and stable. Prairie dog burrows were
found within the embankments in both ponds but is more prevalent within pond 9-P8.

The Pump House, Substation and Contractor Staging Area SAEs were inspected. The Pump House and Substation
SAEs are gavel pads. No signs of erosion or sediment leaving the area were observed from these SAEs. The
Contractor Staging Area is well vegetated with a berm surrounding the SAE directing runoff to ditches. No erosion
was observed at the Contractor Staging Area.

Culvert found along the berm and ditch 5-D1 was found partially filled at the inlet. The culvert appears to relieve
runoff from the area to a sump before entering a vegetated buffer. All runoff from this area is now directed to the
perimeter ditch along the shop and warehouse area and eventually to Pond 5-P3.

Culvert 34 is partially filled at the inlet and is in need of clearing.

Mine dewatering pump not discharging.
