Please have your staff send the <br /> necessary information or contact the appropriate permittees. <br /> Additionally, during a recent joint inspection, the Office of Surface Mining Reclamation and <br /> Enforcement (OSM) inspector noted that the operator had not submitted an updated <br /> certificate of liability insurance to AFO. A survey of the Colorado MRPs within the AFO <br /> library revealed that most MRPs still contain the original certificates, which are now long <br /> outdated. A review of the computer tracking of material submitted in 1994 revealed that <br /> no Colorado permittees sent AFO a 1994 insurance certificate of liability insurance. <br /> Colorado regulation 2.03.9 mandates that each permittee submit a certificate of liability <br /> insurance "for inclusion in the permit application," which is the MRP, and that the policy be <br /> maintained for the "life of the permit and any renewal thereof." Therefore, each year's <br /> certificate revises the MRP and should be sent to AFO to update our copies.
