7. Failure to maintain sediment pond 007, which was filled with
sediment.
8. Coal spillage onto the undisturbed area between the Mine No. 1 coal
stockpile and the slurry pond.
9. Non-coal waste improperly disposed in the valley east of Mine No. 1
and at the Mine No. 1 and 2 sediment ponds.
10. Gullying in the embankment below the drainage ditch to the Mines 1
and 2 sediment ponds.
11 Disturbed area drainage bypassing and eroding the embankment of the
south Mine No. 3 sediment pond.
12. Oil and grease spillage from the Mine No. 3 hoist house.
13. Failure to maintain the berm/ditch in the road to Mine No. 3 that
directs drainage to the south sediment ponds.
14. Downslope spoilage of waste over the Mine No. 3 north outslope.
15. Failure to maintain the drainage ditch on the north side of the
Mine No. 3 bench.
16. Coal spillage from the access door of the covered conveyor between
the Mine No. 3 middle and lower bin pads.
17. Failure to monitor oil and grease at discharge points.
18. Failure to adequately salvage topsoil at the site of the two
temporary rock tunnel disposal area sediment ponds.
19. Failure to maintain the drop box at the rock tunnel .
20. Failure to maintain the Mine No. 5 haul road culverts at the
crossing of Oil Well Creek and the Coal Creek Crossing near the old
town of Coal Basin.
21 . Failure to maintain the drainage control berm along the Mine No. 5
haul road.
