No.:Ice of Viola::
Attachment A
Portions of the Ooeration to which this Notice .Acelies :
Road C
1 . The drainage ditch along the entire road does not meat design
requirements . The ditch is intermittently scoured and filled in with
sediment or large rocks . Rock checks required by the approved plan have
not been installed . In some locations , bedrock outcrops have prevented
ditch excavation.
2. At Sta. 95+00 (approximate) , road drainage has been intercepted by an old
pilot road which runs above and parallel to Road C to its point of origin
at Sta. 54+00. Uncontrolled discharge from the pilot road has resulted in
erosion above Road C and sediment deposition in Road C drainage ditch at
multiple locations , including cut slope gullies at 82+00 and 63+50.
3. Evidence that road ditches had been overtopped with resulting rill
erosion on road surfaces and fill slopes was observed between Sta. 122+00
and RO-3 outfall at 92+00, from 92+00 around switchback to 83+00, between
76t00 and 71+00, and around the switchback between 36+00 and 34+00.
Road DA
4. Drainage ditch from Road D to Mine 2 bench does not meet approved
design. The ditch was intermittently scoured and filled in with
sediment. There was some rilling on the road surface , and it appeared
that some drainage from the mine bench may have overtopped a low berm and
run down the road .
Road D
5. The drainage ditch along the entire road does not meet design
requirements . The ditch is intermittently scoured and filed in with
sediment or large rocks . Rock checks required by the approved plan have
not been installed. In some locations , bedrock outcrops may have
prevented ditch excavation.
6. At Sta. 117+00, a culvert whicI passes beneath the road and conveys
drainage from above to a "BAT" pond series below the road fill partially
blocks the drainage ditch and has resulted in overtopping of the ditch.
7. The road surface between the Mine 1 Bench and Sta. 100+00 is sloped to
the outside , and the road fill along this segment is heavily Tilled .
-1 -
