Legal, Financial , and Compliance Information - Rule 2.03 <br /> 1 . Mid-Continent must update the legal , financial and compliance information <br /> in the permit application. This must include updating of the permit <br /> application form. <br /> Hydrology Description - Rules 2.04.5 and 2.04.7(1) and (2) <br /> 1 . Additional baseline ground and surface water quality and quantity data is <br /> needed to adequately address seasonal fluctuations. Mid-Continent <br /> Resources, Inc. has furnished baseline data for the period October 1983 to <br /> March 1984. Data from March 1984 to October 1984 is still required. Also, <br /> field measurements of pH, EC, and temperature do not accompany the <br /> laboratory analysis. All surface and ground water field measurements <br /> should be submitted as part of the ground and surface water quality <br /> baseline data. <br /> Roads - Rule 2.05.3(3) (c) <br /> 1 . The applicant proposes to use rolling dips rather than cross drain <br /> culverts on the refuse pile access road. The Division does not feel that <br /> use of these structures would minimize disturbance to the hydrologic <br /> balance since traffic on the road would churn up mud whenever water was <br /> flowing. In addition, it is doubtful that these structures could be <br /> properly maintained during winter snow removal . <br /> Mid-Continent must either provide the demonstration required by Rule <br /> 4.03.2(1)(e) for alternative specifications or change the designs to <br /> include culverts at the spacing required by Rule 4.03.2(4) (e) (v) (a) . <br /> 2. It is mentioned in the Geo-Hydro drawings that the existing road from the <br /> wash plant will be upgraded. If this is to be done, the appropriate <br /> information required by Rule 4.03.2 must be submitted. <br /> Ponds, Impoundments, Diversions - Rule 205.3(4) <br /> 1 . The Division suggests that no culverts smaller than 18 inches be used in <br /> constructing the drainage system since smaller sized culverts are prone to <br /> plugging and subsequent failure. <br /> 2. It appears that some undisturbed drainage will enter the "conveyor <br /> diversion ditch" and subsequently enter the lower sediment pond. The <br /> ditch must be designed to prevent undisturbed drainage from entering. <br /> 3. The d50 size of rip-rap for various sections of the temporary and <br /> permanent stream channel diversion must be given along with supporting <br /> calculations. <br /> 4. Several places in the Geo-Hydro report on Stability Analysis, Pile Volume <br /> and Surface water calculations, the consultant suggests something "should" <br /> be done. Mid-Continent must either commit to these suggestions or state <br /> why the measures will not be used. <br />
