terms of 30 Calendar days, if appli ble (see Item 7, Invitation for Bid Instructions).
The attached General Conditions for Purchase Orders and Purchase Order Terms and
Conditions apply to all work.
VENDOR OFFSET INTERCEPT SYSTEM
Pursuant to C.R.S. 24-30-202.4(as amended),the State Controller may withhold debts owed to
state agencies under the vendor o set Intercept system for: (a) unpaid child support debt or child
support arrearages; (b) unpaid balance of tax, accrued Interest, or other charges specified in
Article 22, title 39, C.R.S.; (c)unpaio loans due to the student loan division of the Department of
Higher Education; (d) owed amounts required to be paid to the unemployment compensation
fund; and (e) other unpaid debts ovring to the State or any agency thereof,the amount of which
is found to be owing as a result of final agency determination or reduced to judgment as certified
by the Controller.
WORK DESCRIPTION
Provide the Division with a bid for the attached seed mixtures: Mine Site Seed (Mixture, Outslope
Seed Mixture, Diversion Mixture, a Woods Rose Mixture. The mixtures must be formulated
and delivered in accordance with th following specifications;
Mine Site Seed Mixture: Please pr vide the mix In two acre bags having 20.08 pounds of Pure
Live Seed per bag. The total seed 7r this order shall be 602.40 pounds PLS packaged in 30
separate bags.
Outs!Me.Seed Mixture: Please p vide the mix in two acre bags having 29.2 pounds of Pure
Live Seed per bag. The total seed for this order shall be 379.6 pounds PLS packaged in 13
separate bags.
Diversion Mixture_ Please provide t e mix in'two bags, containing 32.25 pounds of Pure Live
Seed per bag. The total seed for this order shall be 64.5 pounds PLS packaged In two separate
bags.
Woods Rose Seed: Please provi a the Woods Rose seed In one (1) bag, containing 9
pounds of Pure Live Seed. The to l seed for this order shall be 9 pounds PLS of Woods Rose
seed packaged in one bag.
For the Mine Site and Outelo ms aieed shall be thoroughly re-mixed in the specified

proportions and delivered in bags nsisting bf a two acre batch of seed at the rate specified.
The seed denoted by an asterisk( on the attached table shall be placed in a separate bag
enclosed with each two acre bag,
The Duvet Ilan Mixture shall be thoroughly,pro-mixed In the specified proportions, and delivered
In 2 bags consisting of 32.25 pounds pure live seed per bag.
