May 21, 2012 C- 1982 - 057 /Seneca II -W Mine SLB <br />operator indicated that all of the channels would be repaired this construction season. The lower portions of <br />channels PM2, 2A, and 2B were all in excellent condition. <br />The outfall of Culvert ME I, repaired last year, was in good condition with no sign of downcutting. <br />The repaired section of 006 Gulch, where the outslope slide of the Mine Entrance Haul Road occurred, was in <br />good condition, with the exception of a small area at the upgradient end of the rip rap. Additional material from <br />the road outslope had sloughed into the rip rapped channel and will need to be repaired. <br />ROADS Rule 4.03 ❑ Construction 4.03.1(3). "4.03.2(3) ❑ Drainage 4.03.1(4)14.03.2(4) ❑ Surfacing and <br />Maintenance4.03.1(5) and (6)14.03.2(5) and (6) ❑ Reclamation 4.03.1(7).4.03.2(7): <br />Roads were in good condition and had been recently maintained, with the exception of the water bars <br />along LU5 at the top of the Pond 016 drainage, which needed to be repaired. <br />REVEGETATION— Rule 4.1511 Vegetative Cover; Timing: <br />Overall, revegetated areas were in good condition. Vegetation is diverse and erosion is minimal. A few <br />areas where weeds needed to be treated were observed: reclaimed LU2, the vicinity of culvert ME -1, Pond 017 <br />and T25 area. <br />SLIDES and DAMAGE - Rule 4.12: <br />Major grading activities for slide repair in the vicinity of Pond 016 appeared to be complete. The slope on <br />the north side of Pond 016 had been cut back to final grade, exposing a band of apparently competent material. <br />The repaired slide area between the shrub plot and Pond 016 appeared to be stable, with no cracking or bulging <br />observed. The operator still needs to finish grade the material from Pond 016A near PM 3, replace topsoil, and <br />seed the area. <br />GPS measurements were taken along the realigned J road and along each of the reconstructed drainage channels; <br />the results were compared to the approved configuration in Exhibit 20 -2 of the permit application. There are slight <br />deviations in the road alignment and portions of PM -2, and the location of culvert J2A. The operator will need to <br />submit a revised map with the locations of all of these features accurately depicted. A culvert had been installed at <br />the base of the northern cut slope, above and draining into Pond 016. This culvert was not proposed in the plan <br />approved with TR -73, so design information will need to be submitted for this culvert. The updated map and <br />culvert infonnation should be submitted in the form of a technical revision, and should be submitted within 60 <br />days of this report. <br />The downdrains on the Pit Ramp slide had been constructed in the locations approved in TR -76, but had not yet <br />been lined with rock as described in Attachment 20 -E of the approved permit application package. <br />The Oil Well and Pump House slide areas were both stable with no evidence of recent movement.
