In addition <br />to the ground inspection conducted May 23r aerial photographs of the site taken May 21S were reviewed. <br />Weather conditions were warm and windy; ground conditions were dry. No activities were observed on site, <br />recent activities included repair of the Pond 016A slide area. <br />AVAILABILITY OF RECORDS Rule 5.02.4(1): <br />Records were inspected at the Sage Creek Mine offices on May 23, 2012. All required records were <br />available and up to date. <br />FISH and WILDLIFE Rule 4.18: <br />The game trails around Ponds 016, 017, and T18 all showed signs of recent heavy use by elk, and ten elk <br />were observed crossing the reclaimed G Road near T18. There was a sand hill crane at the outfall flume of Pond <br />005. A red- tailed hawk was circling above Pond 015. <br />HYDROLOGIC BALANCE - Rule 4.05 ❑ Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, <br />4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water <br />Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage —Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Ponds 006, 015, 016 and 017 were discharging clear water. Ponds 005 and 009 held water below the <br />discharge level. The outfall of Pond 009 is leaking, presumably in the riser joint, and should be repaired when <br />water level allows. Pond embankments were stable and vegetated; spillways were functional. Stocktanks T1, <br />T18, and T25 were dry. T1 had been breached and is no longer constructed as a stocktank. T25 also had been <br />breached to prevent saturation of the slope above Pond 017. Stocktanks T5, T20, T26 and T27 both held water. <br />The spring running into Pond 017 was active, but flow was extremely low. The spoil spring above Pond 016 was <br />discharging into the pond via PM -2. <br />The south area drainage channels were in poor condition as a result of a major storm event last season; the <br />Number of Partial Inspection this Fiscal Year: 13 <br />Number of Complete Inspections this Fiscal Year: 4 <br />Page 2 of 9 <br />
