Name and Address of Operator <br /> Mid-Continent Resources formerly operated a mine, and obtained Permit CO-0000396, <br /> permitting discharges at various sites on the mine property. Mining ceased in January, <br /> 1991. No resumption of mining is anticipated at this site. On conclusion of mining, <br /> Mid-Continent began to dismantle and remove surface equipment, machinery and <br /> structures, as part of liquidation of assets and reclamation of the mine site. This has <br /> been completed. In addition, Mid-Continent did reclamation until 1994, when the <br /> Colorado Division of Minerals and Geology (DMG) took over reclamation. <br /> At the time of permit application (October, 1998) completion of reclamation is the only <br /> activity occuring at the site, and all such work is under the control, and is being carried <br /> out by the DMG. Mid-Continent Resources has no control over such activities, and is <br /> not consulted or involved in the DMG's planning process. <br /> Mid-Continent's understanding is that the DMG will complete the remaining reclamation <br /> at the former mine site in 1999. No mining or industrial use is anticipated to occur at <br /> the former mine site upon completion of reclamation. <br /> The U.S. Forest Service, the principal landowner, is engaged in land use planning for <br /> the area. Current and anticipated plans include non-motorized recreation, wildlife <br /> habitat and grazing allotments. This applies to areas formerly used by Mid-Continent in <br /> connection with the mining operation, which have now passed into ownership of the <br /> U.S. Forest Service, including the former mine portals at Mines #1, #2, #3, #4 and #5, <br /> and associated benches and haul roads, as well as the areas including refuse piles and <br /> yard area. <br /> For the remaining property within the former mine area, which has been acquired by <br /> Mid-Con, L.L.C., anticipated post-reclamation uses include recreation, such as <br /> horseback riding, cross country skiing and possibly cat served skiing. No further <br /> mining or industrial use is anticipated. Mid-Con, L.L.C., a real estate holding company, <br /> has no employees or equipment, and the sale agreement provided that Mid-Continent <br /> Resources would carry on-site maintenance activities, as necessary. This is <br /> anticipated to include water sampling, as necessary, and clean-out of culverts, and <br /> ditches on lands owned by Mid-Con, L.L.C. Consequently, Mid-Continent Resources <br /> is applying for the permit renewal as "operator" of for those outfalls and compliance <br /> points that remain on Mid-Con, L.L.C. land, and for limited monitoring and maintenance <br /> activities. <br /> As to the property owned by the U.S. Forest Service, including those areas on which <br /> mining activity was previously conducted, the DMG has been conducting reclamation <br /> for some years. DMG has substantially reconfigured the land surface and altered <br /> drainage patterns and sediment control systems previously established by Mid- <br /> Continent. The roads to Mines #3 and #5 have been reclaimed, including removal of <br /> major culvert crossings, such that equipment access to these sites is infeasible. With <br />
