September 23, 2011 C -1981 -014 /Southfield Mine JHB <br /> <br /> <br />Number of Partial Inspection this Fiscal Year: 2 <br />Number of Complete Inspections this Fiscal Year: 1 <br /> <br /> Page 3 of 10 <br /> <br />events fail to clear the sed iment out of the culvert, Energy Fuels will need to clear this material out. <br /> <br />Sediment near the riser pipe on the east end of Pond 5 has been dozed to a more evenly spread o ut configuration. <br />The current configuration of the pond has roughly 8 feet from the elevation of the sediment in the bottom of the <br />pond to the discharge elevation. <br /> <br />Pond 4 was dry with no concerns regarding the discharge structure, embankment, or sedimen t el e vation contained <br />within the pond. <br /> <br />Roadside ditches and permanent ditches at the RDA remain in good condition. <br /> <br />OTHER (SPECIFY): <br /> <br />PROCESSING WASTE/COAL MINE WASTE PILES – Rule 4.10 and 4.11 Drainage Control; Surface <br />Stabilization; Placement: <br /> The vegetative cover on the refuse pile remains good. One of the surface landowners, Ms. Linda Saunders, <br />has expressed concerns over rilling on the refuse pile (e -mail dated9/19/2011). The inspector found no problems <br />on the pile. One very short, approximately 15 feet long, and shallow flow pattern was observed on the north side <br />of the RDA, to the West of Pond 4. Based on the amount of vegetation in the bottom off this shallow rill, and lack <br />of sediment on the vegetation, this rill does not appear to be active ly moving sediment, and appears to be healing. <br />Since the Division and the operator are unable to locate the area of concern that Ms. Saunders has identified, the <br />landowner may pin -flag or otherwise mark the location of her concern. An area that appears t o have been <br />previously repaired on the east side of the RDA remains bare. Additional seed in this locale will help minimize <br />fresh down -cutting in this repaired area. <br /> <br />ROADS – Rule 4.03 Construction 4.03.1(3)/4.03.2(3) Drainage 4.03.1(4)/4.03.2(4) Surfa cing and <br />Maintenance4.03.1(5) and (6)/4.03.2(5) and (6) Reclamation 4.03.1(7)/4.03.2(7): <br /> The roads through out the mine site remain well graded and in good condition. Map 19 shows the <br />permanent and temporary roadways in the permit. <br /> <br />RECLAMATION SUCCESS - Rule 4.15, Rule 3: <br /> The mine reclamation remains stable and approaching the end of the 10 year liability period. The re - <br />established vegetation community remains healthy, though due to dry conditions, the grasses are crunchy <br />underfoot. <br /> <br />Additional obs ervation of the proposed Ponderosa -Pinyon -Juniper reference area ide n tified a previously disturbed <br />& reclaimed roadway either within the proposed reference area or just outside of the proposed area. If this feature <br />falls within the proposed reference area , the operator would need to assure that this previously disturbed featu re is <br />not sampled for reclamation success determination. No marker delineate the boundaries of the proposed reference <br />area. <br /> <br />The grassland reference area was observed. This site, “Reference Plot A” is an open short -grass savannah,
